A FEW REMINDERS
• Emergency Numbers: There is nothing more frustrating than having a sick child and not having a way to contact a parent. Please keep your phone numbers updated and have the phone number of a reliable relative, friend, or neighbor on file in the clinic.

• Medication: A student who must take medicine (prescription or non-prescription) during the school day must bring a written request from a parent and must bring the medicine directly to the clinic upon arrival to school. All medications (prescription and over-the-counter) are required by law to be in their original container and/or packaging box or bottle.

The written request must include: 1.) your child’s full name, 2.) the name of your child’s teacher, 3.) the name of the medication, 4.) the amount of medicine to be given, 5.) the time the medicine is to be given, and 6.) a parent signature. Temporary medications need to have a note initially and then a new note each week as long as medication is to be taken at school. Medications for chronic conditions (asthma, headaches, allergies, etc.) that require medicine to be kept at school need to have one note for my files that states medication is for all year. And yes…I must even have a note for cough drops!
